Ecancermedicalscience 2023:17:1594. This is the first description of the cancer burden in older adults living in the low-and-middle income countries, where the population is ageing the fastest. The number of new cancer diagnoses is expected to double between 2020 and 2040, from 6.3 million to 11.5 million, with the greatest increase expected in lower-income countries. However, healthcare systems are not prepared yet.

• Radkiewicz C, Järkvik Krönmark J, Adami HO, Edgren G. Declining Cancer Incidence in the Elderly: Decreasing Diagnostic Intensity or Biology? Cancer Epidemiol Biomarkers Prev 2022:31:280–6. This large population-based cohort study using Swedish Cancer Registry and autopsy data, authors confirmed a consistent decline in registered cancer incidence in older adults that completely disappears or is substantially reduced when investigating autopsy-detected incident cancers. These findings suggest that the declining incidence in the oldest adults is mainly due to a reduction in reporting, cancer investigation and access to cancer screening.
