Presence of cholesterol in the hydatid cyst fluid was previously observed in a few studies and found to be associated with cyst degeneration. Cholesterol is reported to have an immunomodulatory effect and to be one of the non-immune inducers of inducible nitric oxide synthase(iNOS) expression.
Objective: exploring the relationship between the presence of cholesterol crystals in hydatid fluid, viability of protoscolices and expression of iNOS in their hydatid cyst wall.
Material and methods: Cases were divided into two groups: Group 1 included six patients with cholesterol crystals (CCs) in their hydatid cyst fluid and group 2 included 12 patients without CCs in their hydatid cyst fluid. The viability of protoscolices inside the hydatid cysts was assessed and correlated with the expression of iNOS in the hydatid cyst wall and compared between both groups.
Results: protoscolices viability was markedly affected in group 1 containing cholesterol crystals in their hydatid cyst fluid. The mean viability percent was 4.5±3.9 in group1, while in group 2 it was 35.4±20 with a highly significant statistical difference. High expression levels of iNOS were detected in sections from the hydatid cyst walls by immunohistochemistry. A significant negative correlation was found between the degree of tissue expression of iNOS and the percentage of viability of hydatid protoscolices.
Conclusion: The decreased viability of protoscolices suggests a role for cholesterol in immunomodulation, possibly through the involvement of the nitric oxide pathway.Keywords Key words: Hydatid cyst; iNOS; cholesterol crystals; viability of protoscolices Main Subjects Helminthology
