A 70-year-old diabetic lady presented with acute cholecystitis and later developed pseudomembranous enterocolitis (PME) confirmed by CT (Accordion sign) and stool assay. Treated with metronidazole and vancomycin, she recovered fully. This case highlights PME’s rare imaging features, the importance of cautious antibiotic use, and the role of early diagnosis and intervention.
