Sagittal fractures of maxilla and palate are uncommon in clinical practice whether in the midline or paramedian region, horizontal and comminuted, usually associated with Le Fort fractures. Initial stabilization of these fracture prevents the functional impairments and life-threatening situations, such as continuous haemorrhage from the traumatized nasal mucosa and nasal regurgitation of food leading to the risk of aspiration, immediate closure of the oroantral communication, close approximation of the lacerated palatal mucosa, and improved voice quality & does not hinder intubation procedures to secure airway.
PurposeThe purpose of this technique is to provide management of palatal fractures in emergency or casualty basis & to prevents the functional impairments and life-threatening situations.
MethodThe authors present the simple and practical technique of bilateral transmolar/ transpalatal wiring technique with use of limited armamentarium and wires to manage these palatal fractures. Current method of transpalatal wiring for the management of palatal fractures have advantages and limitations.
ConclusionThe transmolar approach is simple and provide adequate stabilization in emergency management of palatal fractures preventing further complications. This is only the preliminary report of transpalatal wiring technique further prospective clinical trials are required to compare our technique with the preexisting techniques.
