Heller myotomy with esophageal diverticulectomy was standard of care for achalasia concomitant with epiphrenic diverticulum. With increasing frequency, POEM with diverticulotomy has been demonstrated as a feasible method for management of achalasia with a paraesophageal diverticulum. Here, we present two patients with achalasia and paraesophgeal diverticulum that have symptoms refractory to Heller myotomy.
