Managing the airway in a mid-face deformity due to invasive aspergillosis can be challenging even for an experienced anesthesiologist. Bag and mask ventilation is impossible due to the necrosed and deformed upper airway. Proper patient counseling and explanation for awake tracheal conversion to submental intubation remain prerequisites. The patient will be aware of the technical problems encountered during airway securement due to necrosed and disfigured face. This report highlights the importance of adequate patient preparation and the expertise of the anaesthesiologist in dealing with a problematic airway due to a deformed facial fungal infection.
