The guest editors of this issue of Seminars in Neurology are Drs. Michael Schneck and José Biller.

Dr. Schneck is Professor of Neurology (and Neurosurgery) and Vice Chair of the Department of Neurology for Clinical Research and Faculty Development of the Stritch School of Medicine at Loyola University. His areas of expertise include cerebral aneurysms, intracerebral hemorrhage, brain arteriovenous malformations, strokes, CADASIL (cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy), carotid stenosis, and moyamoya disease. He obtained his MD from Weill Cornell Medical College, followed by internship and residency in neurology at the Hospital of the University of Pennsylvania and Temple University Hospital, followed by fellowship at Yale University. He is the Editor of Frontiers of Neuro-critical and Neurohospitalist Care, and an editorial board member and reviewer for many other national and international journals. He has published more than 125 peer-reviewed articles and has been involved in guidelines, book chapters, and other scholarly activities including multiple clinical trials. He has served as Vice Chair of the United Council of Neurologic Subspecialties (UCNS) Certification Committee, the UCNS Neuro-critical Care Exam committee, and has been involved in other national society committees during his career. In 2021, Dr. Schneck was awarded the Frank A. Rubino Award for Excellence in Clinical Neurology Teaching by the American Academy of Neurology (AAN).

Dr. Biller is Professor of Neurology and Neurological Surgery and Chairperson of the Department of Neurology at Loyola University Chicago Stritch School of Medicine. Dr. Biller served as Director of the American Board of Psychiatry and Neurology (ABPN) from 1994 to 2001, and President of the ABPN in 2001, and currently holds the title of Emeritus Director of the ABPN. He is the Editor of the Journal of Stroke and Cerebrovascular Diseases, the recent past Chief-Editor of Frontiers in Neurology, and an editorial board member and reviewer for an array of other national and international journals and publications. He has published more than 380 peer-reviewed articles, more than 155 book chapters, edited 35 books, and given more than 750 lectures around the world. Dr. Biller earned his medical degree from the School of Medicine at the University of the Republic in Montevideo, Uruguay, where he had post-graduate training in internal medicine. He then completed Residency in Neurology at Loyola University Chicago and a Cerebrovascular Research Fellowship at Wake Forest University, Bowman Gray School of Medicine. Dr. Biller was the recipient of the American Neurological Association (ANA) Distinguished Neurology Teacher Award in 2013. He was also awarded the 2020 A.B. Baker Lifetime Award for Teaching in Neurology by the American Academy of Neurology (AAN).

We greatly appreciate the efforts of Drs. Schneck and Biller, as well as all of the contributing authors, for their wonderful work in this issue of Seminars, and I'm quite certain that all practicing neurologists will find these cases stimulating, and the discussions insightful. This is a new kind issue for Seminars in Neurology, and we suspect you'll find it thought-provoking as well as helpful, as we all face challenging cases in our day-to-day practice, regardless of whether you're a comprehensive neurologist or a subspecialist.
