The Effect of Reiki on Fatigue and Comfort in Hemodialysis Patients

Although hemodialysis is a life-saving treatment, it significantly affects patients' lifestyles and leads to many complications1,2. HD patients face many problems such as dependence on the dialysis machine 2-3 times a week for 3-4 hours, dietary changes, fluid restriction, limitation of social life, insomnia, pain, fatigue, anxiety, changes in body image, and deterioration in sexual function1, 2, 3, 4. These problems cause patients to experience uncertainties about the future and impair their quality of life1,3. Fatigue is seen in 60-97% of patients receiving HD treatment5,6. Fatigue, affecting patients' daily life activities adversely, reduces self-care activities, and can disrupt family and social roles. It also causes unemployment and increased dependence on health care7,8. Therefore, patients can resort to complementary therapies together with the pharmacological treatment to relieve symptoms, strengthen their immune system, support their standard treatment, feel cognitively, physically, emotionally, spiritually and socially well, protect their kidneys, and increase their quality of life and comfort8, 9, 10, 11. One of such complementary methods is Reiki. Reiki is thought as a method that helps restore the balance between body, mind, and spirit by activating the parasympathetic nervous system12,13. Reiki can be applied either from near the patient, or from distance while the patient and practitioner are in separate locations. Both types of Reiki rely on the premise of a universal source of healing energy that a Reiki practitioner can direct through intention14. It also has a significant advantage as it is easy to apply and does not have an adverse effect on behavior and attitudes12,14. Due to the long-term treatment of HD patients, the healthcare team members who spend the most time with them are nurses. Nurses have essential roles and responsibilities in helping patients cope with fatigue and increasing their comfort, as they are in a position where they can be in close communication with the patient, immediately recognize their needs, and intervene quickly1,15. There are studies in the literature in which Reiki was applied to healthy individuals and individuals with chronic diseases, showing that it was effective on symptoms such as pain, depression, stress, anxiety, quality of life, and fatigue16, 17, 18, 19, 20, 21, 22, 23, 24. In the literature, it was determined that Reiki energy application reduced stress and fatigue in nurses working during the COVID-19 pandemic19, it was effective in reducing pain, stress, anxiety, and fatigue in oncology patients20, 21, 22, 23, it improved the quality of life21, it increased the comfort and well-being of patients24, it was effective in improving certain behavioral and memory problems in patients with Alzheimer's disease, and it reduced the symptoms of pain, fatigue, and depression in HD patients25. In the literature, there is only one research that investigates the effect of Reiki on fatigue in HD patients. In this research Reiki was determined to reduce fatigue 25. However, as far as we know, there is no study showing the effect of distant Reiki on the control of fatigue symptoms experienced by HD patients and on the comfort level of such patients. This research was conducted to determine the effects of distant Reiki on hemodialysis patients' fatigue and comfort levels.
