Lyme disease is an infection caused by Borrelia burgdorferi sensu lato (Bb sl), transmitted by the bite of a tick belonging to the genus Ixodes, present throughout metropolitan France [1].

Clinical signs vary according to the phase of the disease. Cutaneous involvement is the most frequent clinical form of Lyme disease [2]. Conclusion

This case shows a rare presentation of Lyme disease with predominantly NAP involvement. Cutaneous abnormalities of the NAP are rare and may correspond to multiple pathologies. The possibility of carcinomatous skin involvement must be systematically evoked. Diagnostic management should not be delayed by trial local treatments and a skin biopsy should be rapidly proposed in the absence of an obvious clinical aetiology.
