Zusammenfassung

Dargestellt werden die Organisation und die Tätigkeit eines Giftnotrufs beispielhaft anhand der Daten aus dem Jahresbericht 2018 des Giftinformationszentrums-Nord. In einer kurzen Übersicht werden die Grundpfeiler der klinischen Toxikologie zusammengefasst: die Indikationen zur primären Giftentfernung (Magenspülung und Aktivkohlegabe), zur sekundären Giftentfernung (Hämodialyse, Hämoperfusion, verspätete und repetitive Gabe von Aktivkohle, MARS) und zur Gabe spezifischer Antidota oder Antivenine (Antiseren bei Intoxikationen durch Gifttiere).

Die Magenspülung ist indiziert innerhalb einer Stunde nach Ingestion einer potenziell letalen Dosis einer Noxe; bei Intoxikationen mit ZNS-gängigen Substanzen in der Regel mit Intubationsschutz. Herausgestellt wird das präklinische Management von Vergiftungen mit den besonderen Implikationen für den Notarztdienst. Vorgestellt wird die „Bremer Liste“ – eine aus fünf Medikamenten bestehende Minimalliste von Antidota für den Notarztwagen (Atropin, 4-DMAP, Toloniumchlorid, Naloxon und Aktivkohle).

Summary

The organisation and work of a poisons centre is demonstrated on the basis of the GIZ-Nord Poisons Centre Annual Report for 2018. The basic principles of clinical toxicology are summarized in a short overview: the indications for gastric lavage and the application of activated charcoal. Moreover, methods to achieve an enhanced elimination are presented: haemodialysis, haemoperfusion, multi-dose activated charcoal and MARS. Gastric lavage is indicated within one hour after ingestion of a life-threatening dose of a poison. In case of intoxications with CNS penetrating substances, gastric lavage should be performed only after endotracheal intubation due to the risk of aspiration. The basic management of the intoxicated patient by emergency medicine personnel both out of hospital and on the way into the hospital is presented. The “Bremen List”, a compilation of five antidotes (atropine, 4-DMAP, tolonium chloride, naloxone, activated charcoal) for the out-of-hospital treat­ment by emergency doctors is introduced.
